How to get it?
1. Search and Download from your App Store: Health4Me
2. Open the App, Click “Register”
3. Enter First Name, Last Name, Date of Birth
4. Select “I am a UnitedHealthcare Member”
5. Then you have 2 options:
- Select “I have my ID Card”
- Type in your member ID: (it’s on your UHC card)
- Group Number is: 703443
- Or Select “I do not have my ID card”
- Type Last 6 digits of SSN#
- US Zip Code of your address
6. Click “Continue”
7. Continue to follow step by step instructions
