Wellness Raffle Reminder for COMANCO Employees
COMANCO employees participating in the company medical plan are reminded to complete their annual physical exam to maintain their wellness discount rate for medical insurance coverage in 2020. This wellness raffle reminder also gives employees the opportunity to enter COMANCO’s Annual Wellness Raffle for cash prizes and gift cards.
Employees who participate in the medical plan and submit their completed 2019 Annual Physical Completion Form before November 30th will automatically be entered into the raffle as part of this wellness raffle reminder program.
Annual Wellness Raffle Information
In December, the COMANCO Human Resources Department will host the Annual Wellness Raffle featuring CA$H prizes and $50 gift cards. This wellness raffle reminder encourages employees and spouses covered under the medical plan to complete their required annual physical forms before the deadline.
Many COMANCO employees still need to complete their annual physical exam. Employees are encouraged to contact Claudia in HR to verify that their completed physical form has been received and properly documented.
If your spouse is covered under the COMANCO medical plan, their completed Annual Physical Completion Form must also be submitted to maintain your wellness discount rate.
This wellness raffle reminder helps ensure employees maintain their medical plan wellness discounts while promoting preventative healthcare.

Please remember that COMANCO only requires the Annual Physical Completion Form. Employees should not submit any personal medical results or confidential health information.
